Please read these before your visit
Office Policies
- Patients under the age of 17 MUST be accompanied by the parent or guardian for ALL appointments
- Patients arriving 10 or more minutes late may have to be rescheduled
- Medication refills are not given to patients who do not keep their follow up appointments
- Patients must allow 1-3 business days for and all C2 prescriptions
- Patients with a balance of over $100.00 can not be seen by telemedicine until the outstanding balance is settled.
- Payment is REQUIRED at the time of service. This includes copayments, deductibles, and previous balances.
- Payment for services is the responsibility of the patient
- This office will not refile claims if the patient has not updated their information and services for future appointments will be have to be in cash until info is current.
- A $25.00 No show Fee may apply for no show appointments
New Patients (ONLY)
- IF the New Patient Paperwork has been completed and RETURNED to our office at least 24 hours in advanced, new patients must arrive NO LATER THAN their appointment time.
- If the paperwork is NOT completed and RETURNED to our office at least 24 hours in advance, the patient must arrive 30-45 minutes early, otherwise, we will reschedule the appointment
- This office does not routinely prescribe XANAX
- New patients are REQUIRED to bring in their current medications or a list thereof to the initial visit
Telemedicine Instructions
- Enter the website address exactly as it appears: https://psot.doxy.me into your Web browser
- Enter your clinicians room name using the drop-down arrow on the right side of the “Who are you here to meet with?”
- Choose either “papajoe”, “mamajo” or “Divina Lewis”
- Enter room after your selection
- You MUST give access to the microphone and webcam to continue with the Telemedicine appointment.
- Enter your name to “Check In:
- Enter Room
- Wait for the Provider to join in on video (This is what is called a virtual waiting room!)
